Jason

This review was commissioned following the tragic death of Jason, who died by suicide in May 2023. The review was undertaken because domestic abuse had featured within both previous and more recent relationships, meeting the criteria for a Domestic Homicide Review (DHR).

The Community Safety Partnership extends its sincere condolences and sympathies to Jason’s family, friends and all those affected by his death. We recognise the profound impact that such a loss can have on loved ones and acknowledge the effect that these circumstances can also have on the professionals involved in providing support and services. The review panel similarly expressed its deepest sympathy to Jason’s family and everyone affected by his death.

DHR's are undertaken to better understand the circumstances surrounding a death, identify learning, and strengthen how agencies work together to support individuals and reduce future risk. This review considered agency involvement with Jason and highlighted learning in relation to domestic abuse, stalking and harassment, suicide prevention, risk assessment, and multi-agency working. The review also contributed to a greater understanding of the links between domestic abuse and suicide which has prompted additional training and awareness events throughout  2026 and into 2027.

Partner agencies have worked collaboratively to respond to the findings and recommendations from the review. The learning has informed improvements across a range of services, including domestic abuse responses, primary care pathways, suicide prevention arrangements, and support for bereaved families, helping to strengthen safeguarding practice and reduce the risk of future harm.
